Vision Care Insurance Section 5.23 Dept: Human Resources Revised: April 16th, 2007

Vision care insurance is available to regular employees who work at least 30 hours per week, for at least 10 months of the year.

Two PPO vision plans are offered to Biola employees.  One plan covers eyewear only (the eye examination must be performed elsewhere at the employee's expense).  Most employees combine the vision exam benefit from their health plan with the eyewear-only vision plan.  The second option is a full-service vision plan.

The eyewear-only plan offers both in-network and out-of-network benefits and allows the employee to receive either glasses or contacts every 24 months.  If the insured's prescription changes after one year, the plan will cover new lenses but not frames.  There is a $95 allowance on frames.  A $10 fee for materials is charged up front.

The full-service vision plan provides an annual eye examination for a $25 copay and allows the employee to purchase either glasses or contacts every twelve months.  The employee receives a $100 allowance for frames and a $135 allowance for contact lenses.  If contact lenses are deemed medically necessary, the employee receives a $250 allowance each year.  Out-of-network benefits are available on a reimbursement-for-services basis.

Vision care insurance is among the benefits offered in the Biola cafeteria plan.  An eligible employee can use cafeteria plan dollars toward the purchase of vision insurance.  The cost of the insurance and the amount of cafeteria plan dollars available will determine whether any employee contribution toward the premium is necessary.
